Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based

Key Responsibilities

• Provide skilled occupational therapy services to home health patients in the Hood River, OR and surrounding areas
• Evaluate and treat patients with diagnoses including dementia, Parkinson's, ALS, MS, orthopedic post-op, and post-stroke conditions
• Complete OASIS documentation accurately and in a timely manner using EPIC Remote Client and Rover mobile application
• Support patients with activities of daily living including functional activities related to acute and chronic disorders
• Visit up to 6 patients per day, with occasional travel radius of up to approximately 60 miles per day
• Adhere to productivity standards and maintain compliance with National Patient Safety Goals and core measures

Required Qualifications

• Minimum 1 year of skilled home health occupational therapy experience
• Active licensure in both Oregon (OR) and Washington (WA) required (pending licensure considered)
• Current Basic Cardiac Life Support (BCLS) certification
• Demonstrated OASIS documentation experience
• Reliable personal transportation required for patient home visits

Preferred Experience / Skills

• Experience with home health billing practices
• Familiarity with National Patient Safety Goals and core measures
• Background working with patients on functional activities related to acute and chronic disorders
• Experience with EPIC Remote Client or similar electronic health record platforms
