Key Responsibilities
- Provide physical therapy services to patients in a skilled nursing facility and assisted living/alcoholism treatment settings
- Assess patients' physical needs and develop individualized treatment plans
- Implement therapy interventions to improve mobility, strength, and overall functional ability
- Collaborate with healthcare team members to ensure optimal patient outcomes
- Document patient progress and adjust treatment plans as necessary
Required Qualifications
- Valid license as a Physical Therapist in the state of California
- Proven experience working in SNF (Skilled Nursing Facility) and AL/IL (Assisted Living/Independent Living) environments
Preferred Experience / Skills
- Experience with rehabilitative therapy in diverse healthcare settings
- Strong communication and teamwork skills
- Ability to work independently and manage a caseload
