Key Responsibilities

• Provide physical therapy services to patients in skilled nursing facilities and assisted living/independent living communities.
• Assess patient needs and develop individualized treatment plans.
• Implement therapy procedures to improve mobility, strength, and function.
• Document patient progress and update care plans accordingly.
• Collaborate with healthcare team members to ensure optimal patient outcomes.
• Maintain compliance with facility and regulatory standards.

Required Qualifications

• Valid state license or equivalent certification as a Physical Therapist.
• Proven experience in rehabilitation or working with elderly populations preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Strong assessment and documentation skills.

Preferred Experience / Skills

• Experience working in skilled nursing facilities or assisted living communities.
• Bilingual abilities are a plus.
