Clinical Social Worker – Transition of Care

Greensboro, NC
Schedule / Job Type
Contract | Full-Time | Onsite | Days | Monday–Friday, 8:00 AM–4:30 PM | Holidays as needed
Key Responsibilities
- Manage disposition planning for adult Neurology patients, with a primary focus on Stroke and Seizure populations
- Coordinate Skilled Nursing Facility (SNF) placements and arrange home health and DME services
- Address Social Determinants of Health (SDOH), including domestic violence, substance abuse, and homelessness
- Collaborate within a Transition of Care Department to support safe and effective patient discharge planning
- Float to other campus locations as needed to support varying patient population demands
Required Qualifications
- Active North Carolina clinical social work license required and must be in hand at time of submission
- Minimum 1 year of clinical social work experience
- Basic Life Support (BLS) certification required
- Proficiency with EPIC electronic health records system required
Preferred Experience / Skills
- Licensed Clinical Social Worker (LCSW) with experience in a psychiatric or behavioral health setting strongly preferred
- Familiarity with SNF placement processes and home health or DME coordination
