Key Responsibilities

• Provide direct patient care in a long-term care and skilled nursing facility setting
• Administer medications and treatments as prescribed by physicians
• Monitor and document patient health status and changes in condition
• Collaborate with interdisciplinary care teams to support resident care plans
• Ensure compliance with facility policies, procedures, and state nursing regulations

Required Qualifications

• Active and unrestricted LPN or LVN license in the state of South Dakota or compact equivalent
• Experience in a long-term care or skilled nursing facility environment
• Ability to work day shift hours within a Sunday through Saturday work week
• Ability to fulfill guaranteed hours of 36 hours per week (12-hour shifts) or 40 hours per week (8–10 hour shifts)

Preferred Experience / Skills

• Prior experience in a SNF or LTC setting with geriatric patient populations
• Strong documentation and electronic health record skills
