Contract | Days | Full-Time | 5x8-Hour Shifts (8:00–16:30) | Onsite

Key Responsibilities

• Provide care coordination and case management services for inpatient populations, including neurological and cardiac patients
• Collaborate with interdisciplinary teams within a specialty hospital setting, including a comprehensive stroke center
• Manage caseload utilizing Epic electronic health record system
• Provide weekend coverage with a minimum of 2 weekend days per month (typically one Saturday and one Sunday, not in the same weekend)
• Float to other campus locations as needed based on census levels

Required Qualifications

• Minimum 1 year of clinical experience
• Active BLS certification required
• Active LSWAIC licensure required
• Inpatient and/or acute care experience required
• Case management experience required
• Proficiency with Epic electronic health record system required

Preferred Experience / Skills

• Prior experience in a specialty hospital environment, particularly neurology or cardiology settings
• Experience working within a comprehensive stroke center
