Key Responsibilities
- Provide direct nursing care to residents in a long-term care and rehabilitation setting within a 71-bed skilled nursing facility
- Manage and monitor patient care for a general long-term care population as well as residents on a dedicated rehab wing
- Collaborate with TMAs and other nursing staff, maintaining patient ratios that vary between 2–3 patients per nurse during day shifts
- Participate in every-third-weekend and every-third-holiday rotation
- Document and maintain accurate patient records using the PCC electronic medical records system
- Complete facility-specific orientation by shadowing an experienced nurse during the first shift
Required Qualifications
- Active and unrestricted Registered Nurse (RN) license valid in the state of Minnesota
- Experience in long-term care or skilled nursing facility (LTC/SNF) settings
- Ability to work 12-hour day shifts with every-third-weekend and holiday rotation
- Proficiency with or ability to quickly learn the PCC electronic medical records system
Preferred Experience / Skills
- Prior experience caring for general long-term care populations and post-acute rehabilitation patients
- Familiarity with TMA-assisted care team models in a SNF environment
