Contract | Full-Time | Onsite | Days, Monday–Friday, 8:00 AM–4:30 PM | Holidays as needed | Floating possible based on patient population needs

Key Responsibilities

• Manage disposition planning for adult Neurology patients, with a primary focus on Stroke and Seizure populations
• Coordinate Skilled Nursing Facility (SNF) placements and arrange home health and DME services
• Address Social Determinants of Health (SDOH), including domestic violence, substance abuse, and homelessness
• Collaborate within a Transition of Care Department to support safe and appropriate patient discharge planning
• Assist with other patient populations as needed while maintaining core social work responsibilities
• Float to other health system campuses as required to meet patient population needs

Required Qualifications

• Active Licensed Clinical Social Worker (LCSW) licensure in the state of North Carolina — must be in hand at time of submission
• Minimum of 1 year of social work experience
• Basic Life Support (BLS) certification required
• Proficiency with EPIC electronic medical records system
• Valid driver's license required
• No open background check cases permitted

Preferred Experience / Skills

• Previous experience as an LCSW in an acute care setting strongly preferred
• Familiarity with SNF placement processes and home health or DME coordination
• Experience working with Neurology patient populations, including Stroke and Seizure
