Contract | Days | Full-Time (5x8-hour shifts, 8:00 AM – 4:30 PM) | Onsite / Field-Based

Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting across Hood River, OR and surrounding areas
• Visit up to 6 patients per day, meeting a standard productivity expectation of 30 points per day
• Complete OASIS assessments and documentation accurately and in a timely manner
• Chart using EPIC Remote Client for Home Health (laptop) and Rover mobile application
• Manage daily patient visits with independent transportation, with an occasional maximum radius of approximately 60 miles and up to 90 minutes of total drive time per day

Required Qualifications

• Active Oregon (OR) and Washington (WA) Physical Therapist licensure — both licenses required
• Minimum of 1 year of skilled home health experience
• OASIS experience required
• Basic Cardiac Life Support (BCLS) certification
• COVID-19 primary vaccination series plus one booster required
• Must have reliable personal transportation

Preferred Experience / Skills

• Familiarity with EPIC Remote Client and Rover (phone app) charting platforms
• Experience meeting productivity point expectations in a home health environment
• Comfort with independent scheduling and self-directed patient routing across a geographic territory
