LVN / LPN – Hospital Float (LTAC)

Contract | Nights (3x12 Hours, 7:00 PM – 7:30 AM) | Onsite | Every Other Weekend Required
Key Responsibilities
- Float across multiple units including pulmonary care, ventilator care, subacute rehabilitation, long-term care, and memory care units
- Assess patient status, implement clinical nursing interventions, and develop individualized patient care plans to improve outcomes
- Provide long-term and subacute care for patients with varying acuity levels including post-operative, post-stroke, post-cardiac, wound care, dementia, and complex respiratory conditions
- Manage and monitor patients requiring ventilator support, responding appropriately to ventilator alarms following required specialized training
- Recognize and deliver individualized care based on each resident's routines, preferences, and clinical needs, particularly within memory care and dementia units
- Adhere to strict time and attendance policies, including required holiday rotation and blackout date availability
Required Qualifications
- Valid Maryland or Compact LPN/LVN license (required at time of submission)
- Minimum of one (1) year of LPN/LVN experience
- Current ACLS and BLS certifications required
- Ability to float within the facility across multiple units, within scope of practice
- Must be available to attend all orientation days (Tuesday–Friday, day shift) as scheduled
- Must work every other holiday as required (New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas)
Preferred Experience / Skills
- Prior experience in long-term care, subacute rehabilitation, pulmonary care, or ventilator care settings
- Experience caring for patients with dementia, complex respiratory conditions, or chronic tracheostomy needs
- Strong organizational and leadership skills with the ability to thrive in a fast-paced, high-acuity environment
