Contract | Days Shift (6:30 AM – 7:00 PM) | 0.9 FTE | Every third weekend and holiday rotation required | Onsite

Key Responsibilities

• Utilize the nursing process and collaborate with interdisciplinary team members to deliver patient-centered care in a skilled nursing setting.
• Administer prescribed medications and treatments in accordance with approved nursing techniques, policies and procedures, standards of care, and applicable State laws and practice acts.
• Develop, initiate, and execute individualized patient plans of care as prescribed by the physician and/or facility policy, including patient and family instruction.
• Document patient care activities and outcomes in accordance with established standards of care.
• Participate in department or unit performance improvement activities, which may include shared governance initiatives.
• Apply critical thinking skills to support patient safety and experience, escalating concerns through the appropriate chain of command as needed.

Required Qualifications

• Current, active Registered Nurse (RN) license in the state of Wisconsin.
• Current CPR certification.
• Education verification required.
• Prior experience in a skilled nursing or long-term care environment.
• Ability to float to other inpatient units based on census needs.

Preferred Experience / Skills

• Experience working in a sub-acute or post-acute care setting.
• Familiarity with interdisciplinary care team collaboration and longitudinal care planning.
