Contract | Days (5x8 Hours, 8:00–17:00) | Onsite / Home Health

Key Responsibilities

• Provide physical therapy assistant services to home health patients across an assigned territory (40–80 mile radius)
• Perform patient assessments including strength testing, pain assessment, sensory response evaluation, and skin integrity checks
• Implement and instruct patients and families on home exercise programs, body mechanics, and adaptive/assistive device use
• Assist with therapeutic interventions including gait training, transfers, lifting techniques, and range of motion exercises (AROM/AAROM/PROM)
• Collaborate with supervising Physical Therapist and interdisciplinary team to coordinate patient care plans
• Manage scheduling, supply ordering, patient/family education, and documentation using EPIC

Required Qualifications

• Active Physical Therapy Assistant (PTA) state license in Illinois (license in hand required at time of submittal)
• Minimum of 2 years of experience as a PTA; first-time travelers not accepted
• Current BLS certification
• Previous home health care experience required
• Proficiency in clinical skills including vital signs, breathing techniques, total hip/knee replacement protocols, supplemental oxygen, and use of assistive devices (gait belt, rolling walker, standard walker, sliding boards)
• Candidate must reside at least 75 miles from the facility; local candidates are not accepted

Preferred Experience / Skills

• Experience in cardiopulmonary, geriatrics, medical-surgical, and orthopedic patient populations
• Strong critical thinking, organizational, and time management skills with the ability to work independently in a home health setting
• Familiarity with EPIC electronic health records charting system
