Contract | Days, Monday through Friday | 32–40 hours per week
Key Responsibilities
- Provide skilled nursing care to patients in their home environment within the designated service area
- Perform OASIS assessments and develop individualized care plans for in-home patients
- Monitor and evaluate patient progress, adjusting care plans as clinically appropriate
- Participate in the on-call rotation, including nights, weekends, and holidays on a one-in-four schedule
Required Qualifications
- Active Registered Nurse (RN) license
- Minimum of 2 years of experience performing OASIS assessments
- Experience with care planning for in-home or home health patients
Preferred Experience / Skills
- Experience working with Epic electronic health records
- Prior home health nursing experience in a similar service area or rural setting
