Contract | Full-Time | Days (5x8 Hours, 8:00–16:30) | Onsite / Field-Based
Key Responsibilities
- Provide skilled occupational therapy services to home health patients in Medford, OR and surrounding areas
- Visit up to 6 patients per day, meeting a standard productivity expectation of 30 points per day
- Complete OASIS assessments and documentation accurately and in a timely manner using EPIC/OASIS charting systems
- Manage a caseload with efficient scheduling to minimize drive time across the service area, with occasional travel up to 50+ miles
- Deliver 1:1 patient care in accordance with individualized treatment plans and home health standards
Required Qualifications
- Minimum of 1 year of skilled home health occupational therapy experience required
- Active or pending Occupational Therapist license for the state of Oregon
- Current Basic Cardiac Life Support (BCLS) certification
- Proficiency with OASIS documentation
- COVID-19 primary vaccination series plus at least one booster required
- Reliable personal transportation required
Preferred Experience / Skills
- Experience with EPIC electronic health records system
- Familiarity with productivity point-based caseload management in a home health setting
- Comfort working independently across a broad geographic service area
