Key Responsibilities
- Provide physical therapy services in an inpatient setting as the primary area of focus
- Conduct home health visits as part of a combined caseloadutilize company-provided vehicles for home health patient visits
- Document patient care accurately using multiple electronic medical record systems
- Develop and implement individualized treatment plans to support patient recovery and functional goals
Required Qualifications
- Active Physical Therapist license in good standing
- Experience working in inpatient and/or home health settings
- Proficiency with electronic medical record platforms including EPIC, Net Health, PCC, and/or Maestro
- Valid driver's license and ability to operate a company vehicle for home health visits
Preferred Experience / Skills
- Prior experience in both inpatient rehabilitation and home health environments
- Familiarity with multiple EMR systems used across care settings
