Contract | Days | Full-Time | Onsite | 5x8-Hour Shifts (8:00 AM – 4:30 PM)

Key Responsibilities

• Manage a caseload of up to 35 patients within an acute care facility setting
• Oversee discharge planning for assigned units, with responsibility for up to 2 units on weekends
• Coordinate social services and case management activities to support patient transitions and care continuity
• Participate in every-other-weekend coverage and provide holiday coverage as required
• Float between sister facilities nearby the contracted location as needed

Required Qualifications

• Minimum of 2 years of acute hospital case management experience
• Valid state social work license; compact license must be in hand at time of submission
• Availability for every-other-weekend shifts and commitment to holiday coverage
• Ability to commit to working at least 3 of the following 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
• First-time travelers are not eligible for this assignment

Preferred Experience / Skills

• Prior experience in discharge planning within an acute care environment
• Familiarity with multi-unit coverage and high patient-ratio environments
• Experience working within a large health system or multi-facility network
