Licensed Social Worker – Travel Contract

Contract | 12 Weeks | Monday – Friday, 8:00 AM – 4:30 PM | Onsite

Key Responsibilities

• Coordinate management of care and ensure optimum utilization of resources, service delivery, and compliance with external review agencies
• Conduct comprehensive assessments, develop care plans, implement plans, and evaluate individual patient needs on an ongoing basis
• Integrate case management, utilization review and management, and discharge planning to enhance quality of patient management and satisfaction
• Promote continuity of care and cost effectiveness through coordination across care functions
• Serve as a patient advocate by addressing holistic patient needs, facilitating resource utilization, and leading discharge planning efforts

Required Qualifications

• Active California Licensed Social Worker (LMSW) credential — must be provided at time of submission
• BLS certification through the American Heart Association — must be provided at time of submission
• Minimum of 2 years of recent case management and discharge planning experience in a hospital setting
• Minimum of 1 year of overall clinical experience

Preferred Experience / Skills

• Experience with utilization review and management in an acute care environment
• Familiarity with electronic medical record (EMR) systems
• Prior travel contract experience in a healthcare setting
