Key Responsibilities

• Perform and document comprehensive patient assessments, incorporating culture, age, developmental level, and risk factors in accordance with RN scope of practice and facility policies.
• Develop, implement, and continuously evaluate individualized plans of care in coordination with patients, families, and the inter-professional team.
• Administer patient care therapies and procedures with clinical autonomy, recognize and respond to changes in patient condition, and communicate findings appropriately.
• Delegate tasks to team members and monitor the accuracy and completeness of delegated care activities.
• Facilitate admission, transfer, and discharge processes, including timely execution of discharge orders, patient and family education, and coordination with Case Management.
• Actively participate in unit-based quality improvement initiatives including patient satisfaction, fall prevention, pressure ulcer prevention, CAUTI reduction, and documentation auditing.

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of Kentucky.
• Basic Life Support (BLS) certification required.
• Associate Degree in Nursing (ADN or ASN) required.
• Ability to communicate and collaborate effectively with all members of the care team.
• Proficiency with or ability to be trained on computer systems and clinical applications.

Preferred Experience / Skills

• Previous clinical nursing experience in a hospital or acute care setting.
• Bachelor of Science in Nursing (BSN).
