Contract (13 Weeks) | Days | Monday–Friday (makeup visits permitted on weekends) | Onsite

Key Responsibilities

• Provide occupational therapy services in an outpatient setting, with approximately 20% of visits conducted at assisted living facilities
• Attend daily stand-up meetings at 8:30 AM, Monday through Friday
• Manage a caseload ratio of 6–7 patients
• Document patient care using Wellsky charting software
• Travel locally to patient locations, with an expected weekly mileage of approximately 50–75 miles
• Adhere to facility dress code requirements (clean, professional scrubs with closed-toe shoes)

Required Qualifications

• Active Occupational Therapist (OT) license in good standing
• Minimum of 2 years of recent licensed OT experience
• At least 1 previously completed travel assignment required — no new graduates
• Experience with home care / home-based therapy (HomeCare HomeBase or equivalent)
• BLS and First Aid certifications required
• COVID-19 vaccination and booster required per state guidelines (medical/religious exemptions considered on a case-by-case basis; approved exemptions require weekly testing)
• Must live at least 50 miles from the facility to be eligible

Preferred Experience / Skills

• Familiarity with Wellsky electronic documentation platform
• Experience working in or alongside assisted living facility environments
• Comfort with local travel and flexible scheduling to accommodate makeup visits
