Contract (13 Weeks) | Full-Time | Days | 5x8 (8:30 AM – 5:00 PM) | Alternating Weekends

Key Responsibilities

• Provide wound care services to home health patients, managing a caseload of up to 30 points per week
• Complete OASIS documentation including Start of Care (SOC), Resumption of Care (ROC), Recertifications, and OASIS Discharges
• Perform phone triage and on-call coverage one to two weeks per month (5:30 PM – 8:30 AM on weekdays), with rare requirement for in-home visits during on-call hours
• Utilize electronic medical records systems to document patient care accurately and in a timely manner
• Demonstrate excellent time management, professionalism, and customer service in all patient interactions

Required Qualifications

• Active RN license in hand at time of submission
• Minimum 2 years of Home Health Wound Care experience
• Homecare Homebase and OASIS experience required within the last year to be considered
• Experience with on-call and phone triage responsibilities
• Reliable, dependable, and compassionate with strong customer service skills

Preferred Experience / Skills

• Experience with Wellsky EMR
• Wound Care Certification (not required but a plus)
