Key Responsibilities
- Provide direct physical therapy services to patients within an outpatient rehabilitation setting
- Evaluate, develop, and implement individualized treatment plans based on patient needs and goals
- Document patient progress and maintain accurate clinical records in accordance with facility standards
- Collaborate with interdisciplinary team members to coordinate patient care
- Be available to work until 7:00 PM two evenings per week as part of the coverage schedule
Required Qualifications
- Active Connecticut Physical Therapist license required
- BLS/CPR certification required
- Availability to work a 4x10 schedule, Monday–Friday, within a 7:00 AM–7:00 PM coverage window
- Availability to work until 7:00 PM at least two evenings per week
Preferred Experience / Skills
- 3+ years of physical therapy experience preferred
- Experience with Epic EMR is a plus
