Key Responsibilities

• Provide medical social services to hospice patients and their families throughout the final stages of illness across a multi-county service area
• Conduct home visits to patients residing in private residences, skilled nursing facilities, assisted living facilities, and adult foster homes
• Deliver counseling, emotional support, and care coordination in collaboration with an interdisciplinary hospice team
• Support patients and families with pain and symptom management resources, spiritual support coordination, and bereavement services
• Participate in on-call and intermittent visit schedules as part of a comprehensive hospice care model

Required Qualifications

• Active Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure required
• Healthcare Provider Level BLS certification required
• Valid driver's license and personal vehicle with auto liability/property damage insurance as required by law; vehicle must be maintained in good working order

Preferred Experience / Skills

• Previous hospice experience strongly preferred; candidates with this background will be given priority consideration
• Oregon LCSW (Licensed Clinical Social Worker) licensure preferred
• Experience as a social worker in a hospital or medical setting preferred
