Newport, VT | Home Health
Schedule / Job Type
Contract | Days | Friday, Saturday, Sunday, and Monday, 8:00 AM – 6:00 PM | Some emergency on-call may be scheduled
Key Responsibilities
- Conduct home visits, traveling from patient to patient within the assigned service area
- Complete and manage OASIS (Outcome and Assessment Information Set) assessments accurately and in a timely manner
- Provide skilled nursing care in accordance with home health and hospice standards
- Document patient care using the HealthWyse (Careficient) EMR system
- Manage and adapt to changes in daily schedule as patient needs evolve
- Respond to emergency on-call situations as scheduled
Required Qualifications
- Active Registered Nurse (RN) license in good standing
- OASIS experience required
- Home health and hospice experience required
- Experience traveling between patient locations for home visits
- Valid driver's license in good standing
- BLS certification required
Preferred Experience / Skills
- Prior experience with HealthWyse (Careficient) EMR system
- Strong ability to manage a flexible, dynamic patient schedule
- Experience in outpatient or community-based care settings
