Contract | 13 Weeks | Rotating Shifts (Days/Evenings) | 8-hour and 12-hour shifts | Day: 6:00 AM–2:00 PM or 6:00 AM–6:00 PM; Evening: 2:00 PM–10:00 PM | Every other weekend and holiday rotation | Expected 36 hours/week

Key Responsibilities

• Provide direct nursing care to long-term care residents, including those with memory care, dementia, and orthopedic diagnoses
• Administer medications, treatments, and interventions in accordance with the plan of care
• Maintain accurate and timely documentation using the facility's EMR system (PCC)
• Collaborate with the nursing team to maintain appropriate nurse-to-patient ratios across assigned shifts
• Participate in facility orientation, including building-specific protocols and safe lift procedures
• Rotate weekend and holiday coverage on an every-other basis

Required Qualifications

• Active LPN or LVN license in the state of South Dakota or a compact state license
• Must live more than 80 miles from the facility to be eligible for standard travel rates
• Ability to work a rotating schedule including days, evenings, 8-hour, and 12-hour shifts
• Willingness to complete facility-specific orientation (approximately 2–4 hours)

Preferred Experience / Skills

• Prior experience in a long-term care or skilled nursing facility setting
• Familiarity with memory care and dementia patient populations
• Experience with PCC (PointClickCare) EMR system
