Contract | Mid Shift | Rotating schedule with expected hours spanning days and evenings across a two-week rotation

Key Responsibilities

• Provide direct patient care to residents in a long-term care/skilled nursing facility setting
• Administer medications and treatments in accordance with physician orders and facility protocols
• Monitor and document patient conditions, reporting changes to supervising nursing staff
• Maintain valid LPN licensure and adhere to all state and facility compliance requirements
• Wear a photo identification badge at all times while on assignment

Required Qualifications

• Active LPN license in the state of New Hampshire
• COVID-19 vaccination (original or updated series) or a documented religious/medical exemption on file with employer; additional doses required where mandated by law
• Valid state or federal issued photo identification required for facility entry

Preferred Experience / Skills

• Prior experience in a long-term care or skilled nursing facility environment
• Familiarity with mid-shift and rotating schedule workflows
