Contract | Mid Shift | Rotating schedule with expected hours spanning both day and evening shifts across a two-week rotation

Key Responsibilities

• Provide direct nursing care to residents in a long-term care/skilled nursing facility setting
• Administer medications, treatments, and interventions in accordance with care plans and facility protocols
• Monitor and document resident health status, reporting changes to the appropriate clinical team members
• Maintain valid state or federal issued photo identification and wear a facility-issued photo badge at all times while on assignment
• Collaborate with interdisciplinary care teams to ensure quality resident outcomes

Required Qualifications

• Active Registered Nurse (RN) license in the state of New Hampshire or compact equivalent
• Current BLS/CPR certification
• Must have completed the original or updated COVID-19 vaccine series, or present a documented religious and/or medical exemption from employer
• Must possess a valid state or federal issued photo identification

Preferred Experience / Skills

• Prior experience in a long-term care or skilled nursing facility environment
• Familiarity with electronic health records (EHR) systems used in LTC/SNF settings
• Strong assessment and clinical documentation skills
