Schedule / Job Type

• Full-time, Day shifts, 1 shift per week

Key Responsibilities

• Promote and maintain participants’ health through education, counseling, and coordination of preventive and rehabilitative services.
• Assess health needs and oversee the development and implementation of individualized nursing care plans.
• Provide direct nursing care in the home setting and conduct ongoing re-evaluations of patients’ needs.
• Coordinate scheduling, communication, and documentation to ensure seamless delivery of home care services.

Required Qualifications

• Current, valid Registered Nurse (RN) license.
• Valid driver’s license, reliable transportation, and proof of automobile insurance.
• Current CPR certification.
• One to two years of recent hospital nursing or acute care experience; home health and/or long-term care experience preferred.

Preferred Experience / Skills

• Experience with electronic health record (EHR) systems and Microsoft Office applications.
• Strong communication skills, with the ability to handle emotional or stressful interactions professionally.
• Ability to prioritize tasks, manage multiple demands, and follow through effectively.
