• Full-time, Days shift, 3 shifts per week

Key Responsibilities

• Provide patient care in a long-term care / skilled nursing facility setting
• Manage residents with dementia, memory loss, wounds, and require IV therapy
• Administer medications and monitor patient conditions using EMR system
• Follow facility-specific protocols for wound care, IV therapy, and other treatments
• Collaborate with healthcare team to ensure optimal patient outcomes
• Adhere to safety and quality guidelines during all patient interactions

Required Qualifications

• Active RN license
• IV certification (IV-C) required
• Experience with elderly or long-term care residents preferred
• Proficiency with electronic medical records (EMR), specifically PCC

Preferred Experience / Skills

• Prior work in LTC or SNF settings
• Experience managing patients with dementia or memory care needs
• Knowledge of wound care and IV therapy procedures
