- Days, Part-time (3 shifts per week), 12-hour shifts
Key Responsibilities
- Provide skilled nursing care to residents in long-term care or skilled nursing facilities.
- Administer medications and treatments accurately according to care plans.
- Monitor and document resident health status and report any changes to the healthcare team.
- Assist with activities of daily living and support patient comfort.
- Collaborate with multidisciplinary teams to ensure quality patient care.
- Follow facility policies and procedures to maintain compliance and safety.
Required Qualifications
- Valid LVN/LPN license in the state of West Virginia.
- Prior experience in long-term care or skilled nursing facilities preferred.
- Ability to work 12-hour shifts and flexible scheduling including weekends.
- Strong communication and patient care skills.
- Basic understanding of electronic medical records and documentation.
Preferred Experience / Skills
- Previous experience working dayshift in a SNF or LTC setting.
- Flexibility to work weekends or rotate shifts as needed.
