Key Responsibilities

• Evaluate, plan, and administer physical therapy treatments based on licensed provider prescriptions in inpatient, outpatient, or home health settings.
• Utilize modalities and skilled therapy techniques to restore function, improve mobility, relieve pain, and promote patient independence.
• Supervise and collaborate with Physical Therapy Assistants and coordinate with other rehabilitation team members to achieve positive patient outcomes.
• Conduct assessments, interpret results, and develop individualized treatment plans involving patients and their families.
• Monitor treatment effectiveness, adjust plans as necessary, and ensure accurate, timely documentation and billing.
• Adhere to safety procedures and participate in performance improvement activities.

Required Qualifications

• Valid Illinois Physical Therapist License
• Valid driver's license with proof of auto insurance
• Basic Life Support (BLS) certification

Preferred Experience / Skills

• Minimum of 1 year experience in Physical Therapy
• Proficiency with Microsoft Office applications (Word, Excel, Outlook)
• Strong independent work skills, excellent communication, and customer service abilities
