Social Worker – Outpatient Psychosocial Evaluations

Schedule / Job Type
Full-time, Day shift (8:30 AM – 5:00 PM)

Key Responsibilities

  • Conduct psychosocial evaluations for pre/post liver transplant patients.
  • Verify caregivers over the phone and gather necessary case information.
  • Manage a case load of approximately 10 new patients per week.
  • Complete non-billable modules as part of ongoing training required annually.
  • Travel candidates from other facilities eligible for travel contracts if separated for at least 3 months, with active licenses honored.

Required Qualifications

  • Active MSW license (LICSW preferred).
  • Minimum of 1 year of experience in a psychosocial role.
  • Excellent communication skills for verifying caregivers and conducting evaluations.
  • Ability to work independently in outpatient settings.
  • Travel compliance with references covering 1 year (or 6 months for CNAs) within the last 3 years.

Preferred Experience / Skills

  • Prior experience conducting psychosocial assessments for transplant patients.
  • Familiarity with outpatient psychosocial evaluation procedures.
  • Ability to work with inpatient and outpatient care teams as needed.
