Key Responsibilities
- Provide skilled nursing care to residents in long-term care or skilled nursing facilities
- Assist with patient assessments, medication administration, and treatment plans
- Document patient care and maintain accurate health records
- Collaborate with interdisciplinary team to ensure quality patient outcomes
- Follow facility protocols and ensure compliance with healthcare regulations
Required Qualifications
- Valid LVN or LPN license
- Current CPR certification
- Proven experience working in long-term care or skilled nursing facilities
- Ability to work 12.5-hour shifts
- Strong communication and teamwork skills
Preferred Experience / Skills
- Experience with electronic health records (EHR) systems
- Prior experience in a SNF or LTC setting
- Knowledge of state and federal healthcare regulations
