Key Responsibilities

  • Provide skilled nursing care to residents in long-term care or skilled nursing facilities
  • Assist with patient assessments, medication administration, and treatment plans
  • Document patient care and maintain accurate health records
  • Collaborate with interdisciplinary team to ensure quality patient outcomes
  • Follow facility protocols and ensure compliance with healthcare regulations

Required Qualifications

  • Valid LVN or LPN license
  • Current CPR certification
  • Proven experience working in long-term care or skilled nursing facilities
  • Ability to work 12.5-hour shifts
  • Strong communication and teamwork skills

Preferred Experience / Skills

  • Experience with electronic health records (EHR) systems
  • Prior experience in a SNF or LTC setting
  • Knowledge of state and federal healthcare regulations
