Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient setting
• Evaluate, plan, and implement individualized treatment programs based on patient needs
• Complete required clinical documentation accurately and in a timely manner
• Collaborate with the care team to support patient progress and outcomes
• Maintain compliance with credentialing and facility-specific requirements throughout the assignment

Required Qualifications

• Active Physical Therapist license required; permanent state licensure required
• Prior experience in an outpatient or relevant physical therapy setting
• Ability to meet contract credentialing and quality assurance requirements
• Must be able to commit to a 40-hour guaranteed workweek, with 32 hours during holiday weeks

Preferred Experience / Skills

• Previous experience as a traveling physical therapist
• Familiarity with outpatient rehabilitation workflows and documentation systems
