Contract | Full-Time | Onsite | Monday–Friday, 8:00 AM – 4:30 PM | 40 Hours per Week | No Call
Key Responsibilities
- Provide physical therapy assistant services to patients in a home health care setting
- Implement treatment plans established by a supervising Physical Therapist
- Assist patients with therapeutic exercises and functional mobility activities
- Document patient progress and maintain accurate clinical records in accordance with care standards
- Communicate effectively with the supervising PT and care team regarding patient status and plan of care updates
Required Qualifications
- Active Connecticut state Physical Therapy Assistant (PTA) license required prior to start
- Minimum of 1 year of home health care experience required
- Ability to work independently in a home care environment
Preferred Experience / Skills
- Experience with home health documentation systems
- Strong interpersonal and communication skills for working with patients and interdisciplinary teams
