Key Responsibilities

• Manage an active patient caseload of 25–30 patients across adolescent, adult, and geriatric populations
• Conduct approximately 12–13 patient visits per day in an outpatient clinic setting
• Provide physical therapy treatment for a broad range of orthopedic conditions including post-operative care, spine, and joint pathologies
• Perform fall risk assessments and implement appropriate intervention strategies
• Carry out case management responsibilities in coordination with the care team
• Document patient care and progress utilizing NextGen EMR

Required Qualifications

• Active Physical Therapist license in the state of Arizona
• Minimum of 1 year of clinical experience as a Physical Therapist (new graduates will not be considered)
• Experience treating orthopedic conditions such as ACL tears, rotator cuff injuries, disc herniation, total joint replacement, and related diagnoses
• Proficiency or ability to work within NextGen EMR
• Ability to manage a high-volume caseload in a fast-paced outpatient environment

Preferred Experience / Skills

• Experience with fall risk assessment protocols
• Background in case management within an outpatient therapy setting
• Familiarity treating across multiple age groups including adolescent, adult, and geriatric patients
