Contract | Full-Time | Onsite | Days, Monday–Friday 8:00 AM – 4:30 PM

Key Responsibilities

  • Triage emergent situations independently, applying advanced nursing knowledge and skills to manage complex clinical scenarios.
  • Monitor acute and chronic medical conditions in coordination with Primary Care Providers (PCPs).
  • Utilize care plans independently to provide both clinic-based and home-based direct nursing care.
  • Administer treatments, procedures, and medications as prescribed to participants.
  • Conduct regular home visits to perform wound care, lab draws, nurse assessments, and other nursing tasks as ordered by the PCP.
  • Collaborate with licensed and non-licensed personnel to support participant care and communicate with staff, caregivers, and external organizations as needed.

Required Qualifications

  • Associate's degree in Nursing required; RN diploma or degree from an accredited nursing program required.
  • Active Registered Nurse (RN) license in the state of Michigan.
  • Minimum of one year of experience working with frail, elderly, or long-term care populations.
  • Current CPR certification through the American Heart Association (AHA), or willingness and ability to obtain upon hire.
  • Must possess a valid Michigan driver's license, reliable transportation, and current auto insurance.
  • Must be 18 years of age or older and fully vaccinated (or willing to be vaccinated) against Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B.

Preferred Experience / Skills

  • Bachelor of Science in Nursing (BSN) preferred.
  • Minimum of one year of home care experience.
  • Minimum of one year of wound care experience.
