Licensed Master Social Worker (LMSW) – Home Health

Contract | Full-Time | Days (5x8 Hours, 8:00–16:30) | Onsite / Field-Based

Key Responsibilities

• Conduct home health visits with patients in the East Portland area and surrounding regions, with a potential of up to 6 patient visits per day
• Complete OASIS assessments and documentation accurately and in a timely manner
• Perform clinical charting using EPIC Remote Client for Home Health and the Rover mobile application
• Provide skilled social work services in a 1:1 patient care setting within a home health environment
• Manage daily scheduling independently, including driving to patient locations within an approximate 60-mile service radius

Required Qualifications

• Active Licensed Master Social Worker (LMSW) licensure in both Oregon (OR) and Washington (WA) required
• Minimum of 1 year of skilled home health experience
• Basic Cardiac Life Support (BCLS) certification required
• OASIS experience required
• Must have reliable personal transportation
• COVID-19 primary vaccination series plus one booster required

Preferred Experience / Skills

• Familiarity with EPIC Remote Client and Rover mobile application for home health charting
• Experience working in a travel or contract home health setting
• Comfort with independent scheduling and managing daily drive time across a broad service area
