Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based

Key Responsibilities

• Provide skilled physical therapy services to home health patients in Medford, OR and surrounding areas
• Conduct patient evaluations, develop individualized treatment plans, and deliver therapeutic interventions in the home setting
• Complete OASIS assessments and maintain accurate clinical documentation using EPIC
• Manage a caseload of up to 6 patient visits per day, meeting standard productivity expectations
• Coordinate care with the interdisciplinary team to support patient goals and outcomes

Required Qualifications

• Active or pending Physical Therapist license in the state of Oregon
• Minimum of 1 year of skilled home health experience required
• OASIS experience required
• Basic Cardiac Life Support (BCLS) certification required
• COVID-19 primary vaccination series plus one booster required
• Reliable personal transportation required; occasional travel radius may exceed 50 miles

Preferred Experience / Skills

• Familiarity with EPIC electronic health records system
• Experience meeting productivity point expectations in a home health environment
