Key Responsibilities
- Provide direct social work services to patients and residents in a long-term care or senior living setting
- Assess the psychosocial needs of residents and develop individualized care plans
- Coordinate discharge planning and community resource referrals
- Collaborate with interdisciplinary care teams to support resident well-being
- Maintain accurate and timely documentation in accordance with facility and regulatory standards
Required Qualifications
- Active and applicable Minnesota Social Work license required
- Bachelor's degree in Social Work from an accredited program required
- Ability to commit to a 13-week contract assignment
- Availability to work 40 hours per week, Sunday through Saturday
Preferred Experience / Skills
- Prior experience in a long-term care, skilled nursing, or senior living environment
- Familiarity with care coordination and discharge planning processes
