Key Responsibilities

• Provide skilled home health nursing care to patients in the East Portland, OR area and surrounding communities
• Conduct patient visits in the field, with up to 6 patients visited per day and a standard productivity expectation of 30 points per day
• Complete clinical documentation using EPIC Remote Client for Home Health and Rover mobile app, including OASIS assessments
• Manage daily patient assignments within a defined geographic area, with occasional drive times of up to 60–90 minutes per day
• Operate independently in a 1:1 care model, exercising sound clinical judgment in a home-based setting

Required Qualifications

• Active Registered Nurse (RN) licensure in both Oregon (OR) and Washington (WA) — required prior to start
• Minimum of 1 year of skilled home health nursing experience
• Basic Cardiac Life Support (BCLS) certification required
• OASIS competency required
• Must have reliable personal transportation
• COVID-19 primary vaccination series plus one booster required

Preferred Experience / Skills

• Experience with EPIC electronic health record system in a home health setting
• Familiarity with the Rover mobile documentation application
• Comfort navigating a wide geographic service area with varying patient locations
