Contract | 13-Week Assignment | Nights (1800–0630) | 12-hour shifts with one 8-hour shift per schedule | Every other weekend | Every other holiday | 80 hours per pay period

Key Responsibilities

• Provide direct nursing care to residents in a long-term care / skilled nursing facility setting
• Monitor and assess patient conditions, with a primary patient population of dementia residents
• Administer medications and treatments in accordance with physician orders and facility protocols
• Document patient care accurately using EMR (Matrix) and paper flowsheets
• Collaborate with interdisciplinary care team to support individualized resident care plans
• Maintain compliance with state and facility standards throughout the duration of the assignment

Required Qualifications

• Active and unrestricted Minnesota LPN or LVN license
• Experience in a long-term care or skilled nursing facility environment
• Ability to work night shifts, including every other weekend and every other holiday
• Familiarity with EMR documentation systems; Matrix experience a plus
• Ability to fulfill a 13-week contract commitment with consistent attendance

Preferred Experience / Skills

• Prior experience working with dementia patient populations
• Comfort working in a 2-story facility with variable nurse-to-patient ratios
• Experience with both 8-hour and 12-hour shift formats
