Contract | Mid Shift | Primarily 8-hour shifts (12-hour shifts considered) | Every other weekend and holiday required
Key Responsibilities
- Provide direct nursing care to residents in a long-term care / skilled nursing facility setting
- Deliver care to a patient population consisting primarily of individuals with dementia
- Utilize EMR and paper flowsheet systems to document and manage patient information
- Administer medications and carry out physician-ordered treatments in accordance with facility protocols
- Collaborate with interdisciplinary care teams to support resident health and well-being
Required Qualifications
- Active Minnesota LPN/LVN license required
- Experience in long-term care or skilled nursing facility (LTC/SNF) setting
- Ability to work every other weekend and every other holiday
- Familiarity with EMR documentation systems
Preferred Experience / Skills
- Prior experience working with dementia patient populations
- Experience with Matrix EMR or paper flowsheet documentation
- Comfort working in a multi-story facility with varying patient ratios
