Home Health LMSW

• Location: Portland, OR
• Schedule / Job Type: Full-time, Days, 8-hour shifts
• Key Responsibilities:
  • Provide skilled home health services to patients in East Portland and surrounding areas.
  • Conduct 1:1 patient visits, with the capacity to visit up to six patients per day.
  • Document care activities using EPIC, Rover, and Oasis systems.
  • Adhere to agency protocols, including COVID vaccination requirements.
  • Maintain productivity standards and comply with travel expectations within approximately 60 miles.
• Required Qualifications:
  • Valid licensure in both Oregon and Washington.
  • Minimum of 1 year of experience in skilled home health.
  • BCLS certification.
  • Experience with Oasis documentation.
• Preferred Experience / Skills:
  • Prior home health experience with Oasis.
  • Ability to work independently managing multiple patient visits daily.

• Travel compensation provided via IRS standard mileage reimbursement.

• Requires primary series COVID vaccination plus one booster.

Travel Compliance: Tier3
Shift times: 8:00 AM – 4:30 PM, Monday through Friday

Submission Requirements:

• List any time-off requests.
• Provide three available interview times.
• Confirm the traveler’s phone number.
• Indicate previous employment with Providence (if applicable).
• Submit two references from within the last three years, covering at least six months of employment.

Note: All travelers must be willing to float within the facility network within the same state. Refusal to float may result in assignment termination or forfeiture of guaranteed hours. Equipment necessary for duties, including communication devices, will be supplied.
