Key Responsibilities

• Provide direct patient care in a long-term acute care setting in accordance with the facility's plan of care
• Cover CNA duties as needed to support unit operations and patient care
• Administer IV therapy and perform other skilled nursing tasks within LPN scope of practice
• Work a flexible schedule including a minimum of every other weekend to meet unit and patient care needs
• Participate in shift coverage Monday through Sunday as assigned; no block scheduling permitted

Required Qualifications

• Active LPN/LVN license (must be on file at time of submission)
• Minimum of 1 year of LPN experience in a long-term care or SNF setting
• Current AHA Basic Life Support (BLS) certification
• Ability to pass a 7-year background check, FACIS Level 3 screening, and NY State check (must be on file at time of submission)
• Reliable personal or rental transportation required; public transportation is not accessible in this area
• Complete work history required, including bed count, EMR used, and facility type for all prior positions; all employment gaps exceeding 90 days must be explained

Preferred Experience / Skills

• Experience with IV therapy administration
• Familiarity with long-term acute care (LTAC) or skilled nursing facility environments
• Experience performing or assisting with CNA-level duties
