Contract | Travel | 13-Week Assignment | Full-Time | Onsite | Monday–Friday, 8:00 AM – 4:30 PM | Projected Start: June 15, 2026

Key Responsibilities

• Evaluate and treat patients admitted to a critical access hospital with a strong rehabilitation focus
• Deliver one-on-one patient care at a rate of one patient per hour in an outpatient and swing-bed rehab setting
• Utilize a fully equipped rehabilitation gym, including adaptive equipment, lift devices, outdoor therapy areas, and a functional kitchen and laundry setup
• Document patient care and progress using the Cerner electronic health record system
• Float to an affiliated long-term care facility located within the same building as needed

Required Qualifications

• Active Physical Therapist state license in good standing
• Current BLS certification required
• Minimum of 2 years of physical therapy experience
• Strong rehabilitation mindset with emphasis on active treatment rather than evaluation only
• Ability to evaluate and treat a diverse inpatient and swing-bed patient population

Preferred Experience / Skills

• Prior experience in a critical access hospital or swing-bed rehabilitation environment
• Familiarity with Cerner documentation platform
• Experience providing therapy in outpatient or multi-setting rehab programs
