Key Responsibilities
- Provide direct nursing care to residents in a long-term care / skilled nursing facility setting
- Complete skilled charting and required clinical documentation during the day shift
- Administer medications and treatments; collaborate with a medication aide during designated hours
- Monitor and care for an elder population including residents with dementia and behavioral health diagnoses
- Participate in a behavioral program supporting residents with conditions such as schizophrenia, bipolar disorder, and paranoia
- Fulfill holiday shift obligations when holidays fall within the contracted block schedule
Required Qualifications
- Active and unrestricted LPN or LVN license
- Experience in a long-term care or skilled nursing facility (LTC / SNF) setting
- Proficiency with electronic medical records (EMR); experience with PCC preferred
- Ability to work 12-hour block schedules including every other weekend
Preferred Experience / Skills
- Experience caring for geriatric populations, including residents with dementia
- Familiarity with behavioral health patient populations in a nursing care environment
- Prior experience with skilled nursing documentation and COVID-related charting protocols
