Key Responsibilities

• Provide social work services within an acute medical hospital setting with an average daily census of 40–50 patients
• Assess patient and family psychosocial needs and develop appropriate care and discharge plans
• Collaborate with interdisciplinary care teams to coordinate patient services and transitions of care
• Participate in weekend rotation to meet departmental needs
• Commit to working 3 of 5 designated holidays as required by the facility

Required Qualifications

• Minimum of 2 years of acute medical hospital social work experience required
• Active California state social work license required (pending licensure not accepted)
• Compact license must be in hand at time of submission if applicable
• Must reside outside a 50-mile radius to qualify as a traveler
• Two professional references covering at least 1 year within the last 3 years required prior to start

Preferred Experience / Skills

• Prior travel contract experience preferred (first-time travelers not accepted for this assignment)
• Familiarity with acute care discharge planning and interdisciplinary team collaboration
