Key Responsibilities

• Evaluate and treat patients in an outpatient physical therapy setting
• Develop and implement individualized treatment plans based on patient needs and clinical findings
• Complete required clinical documentation accurately and in a timely manner in accordance with facility standards
• Collaborate with interdisciplinary team members to support patient progress and continuity of care
• Maintain compliance with credentialing and quality assurance requirements throughout the assignment

Required Qualifications

• Active Physical Therapist license in the state of Illinois (permanent license required — compact license not accepted)
• Experience in an outpatient physical therapy setting
• Ability to meet facility-specific credentialing and documentation requirements prior to assignment start
• Candidates who have worked at this facility in a permanent or PRN capacity within the past year are not eligible for this assignment

Preferred Experience / Skills

• Prior travel therapy experience in an outpatient setting
• Strong documentation skills and familiarity with outpatient EMR systems
• Ability to adapt quickly to facility workflows and protocols
