Key Responsibilities

• Manage a caseload with a 1:35 ratio, providing social work services across assigned units
• Focus on discharge planning to ensure safe and timely patient transitions
• Cover up to 2 units on weekends as needed
• Participate in every-other-weekend scheduling and provide holiday coverage, with an expectation to work 3 of the 5 designated holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Collaborate with interdisciplinary teams to coordinate patient care and community resources

Required Qualifications

• Minimum of 2 years of acute hospital case management experience
• Valid state license required; CA state license preferred — compact license must be in hand at time of submission
• Pending CA license accepted
• At least 1 year of relevant experience required
• 2 professional references covering 1 year of experience within the last 3 years required prior to start
• Must live at least 50 miles from the facility to be eligible as a standard traveler; locals within 50 miles may be considered under separate terms

Preferred Experience / Skills

• Prior travel contract experience preferred; first-time travelers are not accepted for this role
• Experience working in an acute care hospital setting
• Familiarity with discharge planning processes and community resource coordination
