Key Responsibilities

• Provide skilled physical therapy services to patients in skilled nursing facilities and assisted living/independent living communities
• Assess patient needs and develop customized treatment plans
• Implement therapy interventions to improve patient mobility and function
• Monitor and document patient progress and adjust treatments accordingly
• Collaborate with healthcare teams to ensure comprehensive patient care

Required Qualifications

• Valid state license to practice as a Physical Therapist
• Current professional certification in physical therapy
• Experience working with patients in SNF and AL/IL settings
• Strong assessment and communication skills
• Ability to work independently and as part of a multidisciplinary team

Preferred Experience / Skills

• Previous experience in long-term care facilities
• Knowledge of regulatory and documentation requirements for skilled nursing and assisted living
• Bilingual abilities are a plus
